How it works
Who's covered depends on the year of birth. Singapore Citizens and PRs born in 1980 or later are covered automatically from age 30, or from 1 October 2020 if that came later, whatever their health. For those born in 1979 or earlier it's optional. Some who had ElderShield were enrolled automatically in 2021, and since 1 January 2026 others can still join if they don't have a disability already.
A payout starts when an MOH-accredited assessor finds that the person can't do 3 of these 6 activities of daily living:
- washing
- dressing
- feeding
- toileting
- walking or moving around
- transferring, such as from a bed to a chair
The payout is cash, paid monthly for as long as they can't do 3 or more. It's higher the later in life a claim starts, because the amount rises each year until the policyholder turns 67 or makes a claim. The current figures are on CPF's CareShield Life page, and we don't quote them here because they change every year.
Premiums are paid until age 67, or for 10 years after joining if that ends later, and then cover continues for life. MediSave can pay them in full, and a family member can pay from their own account if the policyholder's runs short. The government gives means-tested subsidies, and CPF says no one loses cover because they can't pay. The cover also stays in place if the person moves overseas.
A 2025 review changed benefits, coverage and premiums. The changes began on 1 January 2026 and come in over several years. Private insurers sell supplements on top, for example with higher monthly payouts. Each supplement's terms are the insurer's own.
Worked example, with a made-up client: a man born in 1982 has a stroke at 58. He can feed himself and wash with a rail, but he can't dress, move from bed to chair or walk room to room without help. That's 3 of 6, so an assessor may find him severely disabled, and his monthly payout would start from the claim. The first assessment for CareShield Life is free.
Where it shows up in your week
It comes up when a client's parent needs care, and again at every review of what their cover leaves out. The CRM keeps each client's notes and review dates on their page, so you can pick the conversation up where it stopped.
